Child Health Plus, Family Health Plus, Medicaid Managed Care
These GHI HMO plans are designed to emphasize ready access to affordable, quality care for individuals and families in New York State. They're available for little or no cost in the areas indicated.
GHI has been selected by the State of New York to offer the Child Health Plus program to working families who are ineligible for Medicaid and unable to afford private health insurance. The Child Health Plus program offers primary care services to children (up to age 19). The program is available in: Albany, Broome, Columbia, Dutchess, Greene, Orange, Putnam, Rensselaer, Rockland, Schenectady, Sullivan and Ulster counties.
Available to those without health insurance coverage, this program provides access to top-quality doctors and specialists within GHI's HMO service area. Eligibility for this program is based on household income, residency and age. It is available in: Albany, Broome, Columbia, Delaware, Dutchess, Greene, Montgomery, Orange, Otsego, Putnam, Rensselaer, Rockland, Saratoga, Schenectady, Sullivan, Ulster, Warren and Washington counties.
If you are eligible for Medicaid, you may also be eligible for this program, which provides access to top-quality doctors and specialists within GHI's HMO service area.
