How do I appeal a denied claim by GHI?

If GHI denies a claim that you've submitted, or makes a decision that you believe is unfair, you have a right to appeal that determination.

In reviewing your claim, we may find a service that is not a covered benefit under your GHI plan. If that happens, we will send you a letter of Adverse Determination. That letter will include instructions on how to appeal our decision. Simply follow the instructions to file your appeal.
