The Medicaid Managed Care Program, available through GHI's HMO plans, is a public health insurance program for New York State residents who qualify for Medicaid. For information on Medicaid Managed Care, call New York Medicaid Choice at 1-800-505-5678. You may also visit the New York State Web site at www.health.state.ny.us.
If you would like to continue receiving Medicaid Managed Care from your current doctor, search our provider network or call us at 1-877-244-4466 to determine if your doctor is in the GHI HMO network. Make sure to verify that your provider participates with GHI before scheduling an appointment.
If GHI denies a claim that you've submitted, or makes a decision that you believe is unfair, you have a right to appeal that determination.
In reviewing your claim, we may find a service that is not a covered benefit under your GHI plan. If that happens, we will send you a letter of Adverse Determination. That letter will include instructions on how to appeal our decision. Simply follow the instructions to file your appeal.
